If your child attends TNCS during the regular school year, you need only to provide updated forms if:
- Your child has an Allergy Action Plan or an Asthma Action Plan. These expire at the end of the school year and you must submit new versions for summer camp.
- Your child has had changes to their medical records, such as updated immunizations or a change in medication that TNCS administers.
Please check that you have completed the following forms before your child begins camp.
