OCC Required Paperwork
Maryland State licensing requirements mandate that we collect the paperwork outlined below. This page is for reference only. You will submit your paperwork on Transparent Classroom.

- OCC 1214 Emergency Form
- Required for all students
- Fill out PDF linked above online.
- Complete all items on Part I of the form.
- Sign and date where indicated.
- On Part II, you may make a note that says,”N/A” if no conditions apply. If conditions apply, please detail them.
- If your child has a medical condition which might require emergency medical care, make sure Part II is signed by your child’s health practitioner.
- Required for all students
- OCC 1215 Health Inventory
- Required for all students
- Fill out Part I “Health Assessment.”
- If your child has received a physical examination within the last 12 months, you do not need to fill out Part II. Instead, you may provide a copy of the physical exam paperwork as a substitute for Part II. You should be able to download exam documentation from your child’s Patient Portal.
- If your child has not received a physical examination within the last 12 months, please have Part II completed by a healthcare provider.
- Required for all students
- OCC 896 Immunization Certification / Proof of Immunizations
- Required for all students
- You may complete the form linked above or download proof of immunizations from your child’s Patient Portal.
- Required for all students
- OCC 4620 Blood Lead Testing Certification
- Required for students preschool in through 1st Grade
- You may complete the certificate linked above and have it signed by your child’s healthcare provider or you may download Blood Lead Test Results from your child’s Patient Portal.
- Required for students preschool in through 1st Grade
- TNCS Authorization Form
- Required for all students
- Image Consent & Media Release

- If your child has allergies, asthma, or a seizure disorder, please download the corresponding form(s) below for completion by a healthcare provider.
- Asthma Action Plan and Medication Administration Authorization (OCC Form 1216A)
- Allergy and Anaphylaxis Medication Administration Authorization Plan (OCC Form 1216B)
- Seizure/Convulsion/Epilepsy Disorder Medication Administration Authorization Form (OCC form 1216C)
- Medications: If your child requires ANY medication while on campus (including emergency medications and OTC medicines), submit the Medication Administration Authorization Form.
- The form must be signed by a healthcare provider.
- A separate form is required for each medication.
- If your child has a 504, IEP, or similar plan please include a copy.
- If your child has allergies, asthma, or a seizure disorder, please download the corresponding form(s) below for completion by a healthcare provider.
